Philosophy

The philosophy of Chisholm Trail Hospice is to offer a comprehensive program of palliative and supportive care for the patient and their families during the last phases of terminal illness, including the dying and bereavement stages.  A medically directed interdisciplinary team consisting of patient/families, healthcare professionals, and volunteers provides spiritual, and emotional care physical, social.  Hospice exists in the hope and belief that through appropriate care and the promotion of a caring community sensitive to their needs, patients and their families may be free to obtain a degree of mental and spiritual preparation for death that is satisfactory to them.

  • Services in the home, nursing home, assisted living or hospital

Physicians, nurses, social workers, chaplains, nurse aides and volunteers comprise the core of the interdisciplinary team, which works together with the patient and family to provide care and emotional support. The intimacy of this unique team of health professionals working together creates an environment where it is safe for patients, families, and Hospice personnel to share the precious moments that remain with one another.
